Initiate Parent/Guardian Consent Form

Parent/Guardian Consent only needs to be provided once

• You will need access to your student portal to complete this process.
• Your Parent/Guardian will receive a link to the consent form once you submit the form.  Parents/Guardian will need to sign and submit the link.
